How accurate is my ‘due date’?

MFM mums share their stories plus expert sonographers explain why it happens and what to do if you’re told your due dates are wrong. By Rachel Mostyn. Original date given was 18 December but at the scan it was moved to 31 Dec. Unless you have an early pregnancy scan most of you will be given your estimated due date at the week scan. Of course you have probably already used an online due date calculator. Weird huh? But this will only give you a rough idea. This is especially true if you have an irregular cycle or have been on the Pill for example. Experts say that the scan date can sometimes come out as less pregnant than suggested by your LMP. But then again with my first baby I was spot on with dates, he always measured big and he was a week overdue… these babies like to be tricky!

Scans: what to expect

An ultrasound scan sonogram uses sound waves that bounce off solid objects to create two-dimensional black and white images on a screen. It’s completely safe for you and your baby. It also isn’t completely reliable.

Ultrasound scans use sound waves to create a picture of the baby in the womb. Most women have one or more ultrasound scans during pregnancy. The two scans routinely offered during pregnancy are the nuchal scan and the anatomy scan. Sometimes other scans are performed earlier or later in pregnancy to check the growth of the baby, the position of the baby or the placenta, or for other reasons. Many parts of the body can be investigated by ultrasound but the technique is commonly used to examine the abdominal organs liver and kidney , the pelvis, the heart and the major blood vessels. Other areas which may be examined include the eyes, breasts, and thyroid gland. Most pregnancies are now monitored by ultrasound examination to assess the age, health and position of the unborn baby.

Areas of the body which cannot be successfully examined by ultrasound are those covered by bone, for example the adult brain, and those filled with air, like the lungs. Abnormalities which may show up in ultrasound examinations include cysts, tumours and infections. Blockages in major blood vessels can also be detected.

Ultrasound is a very safe technique and can be used to examine adults, children and babies.

Ultrasound Accuracy for Predicting Due Dates

Every pregnant woman wants to know her due date. But a due date calculated from last menstrual period with a due date calculator often doesn’t match the due date that is estimated by ultrasound also known as a sonogram. During an ultrasound, a technician spreads a warm gel over the lower part of the abdomen, and then presses a tool called a transducer against the belly to examine the fetus using sound waves.

An image of the fetus appears on an accompanying computer screen. While looking at this image, the technician takes some standard measurements from different angles and listens for a heartbeat. Sonograms are risk-free, and seeing your baby will likely be an enjoyable experience.

However, there is some imprecision involved with how due dates are measured. How is a due date calculated? A due date is often a prediction based on your last menstrual period. The calculation is based off of the idea of you having a regular day cycle and that ovulation happens 14 days after your last menstruation.

The due date is calculated by counting days 40 weeks from the first day of the last menstrual period. The caveat is that menstrual cycles are seldom that regular. A normal cycle can be between 21 to 35 days, and some people can even have cycles that range outside of that. If you seek out prenatal care early in your pregnancy, the doctor can perform ultrasounds to more accurately determine the due date.

A screening test can find out if you, or your baby, have a high or low chance of having a health problem. But it cannot usually tell you for certain, so people found to have a high chance of a problem will often be offered a second test.

All women will be offered a dating scan, and an 20 week mid-pregnancy or Measuring the baby can be more accurate than dating by the last menstrual.

Pregnancy math can be a little murky. After all, unless you went the IVF route, your physician has no way of knowing the exact time of conception. Because of this, pregnancy is calculated from the first day of your last menstrual period or LMP. From that day, weeks of pregnancy is officially counted, and your doc or midwife busts out the red pen and marks your due date on the calendar. Sometimes, however, the due date may be revised. It can be pretty difficult to determine a due date using the date-of-last-period method for women with irregular cycles — which is why an ultrasound exam is often required to determine gestational age.

An early ultrasound, usually done at about 6 to 9 weeks, can give the most accurate due date estimation by measuring the size of the embryo or fetus. However, many pregnancies are confirmed by a Doppler heartbeat monitor, not an ultrasound. Did you get your very first ultrasound in your second trimester? If so, that could be why your due date shifted.

Ultrasounds performed during the first trimester not the second give the most accurate due date predictions, according to a study in the American Journal of Obstetrics and Gynecology.
